Referrals for OPG and CBCT Scans

Please use this section to send us OPG and CBCT referrals. Once the form is submitted, we will reach out to your patient asap and schedule them for their imaging visit.

Address and post code - required as per IRMER for patient identification

Patient Phone Number - required to contact the patient for an appointment

Patient Email (if not provided patient will not receive booking confirmation email and details)

Type of image required: We will take a scan according to the area of interest and clinical indications, in line with IR(ME)R and ALARP. The age, anatomy and physical build of patient are also considred.)

Justification for the referral image

Reporting of the Image requested

Not required, the referring dentist will be responsible for reviewing and reporting on the findings.

Required, at extra cost, which will be paid by the patient and the report will be provided by a Radiologist in 10-14 working days.
